Setting up

1. Super Choice form (PDF 154 KB)
   Request your employer to make contributions to your BT Super for Life account. Complete this form and hand it to your employer or payroll manager.
   
   
2. Request to transfer form (PDF 302 KB)*
   Complete this form if you wish to rollover your existing super funds into your BT Super for Life account. Simply complete the details of your existing super fund/s. You will need a new form for each fund you wish to rollover. After completing the form, attach a copy of certified ID. You can certify your ID at your local Westpac branch, Australia Post outlet or Police Station. Make sure you have a separate copy of certified ID for each form you complete. Then send the form/s and your certified ID to your existing super funds.  
   
   
3. BT Super for Life Compliance Letter (PDF 51 KB)
   Provide this letter as evidence of BT Super For Life being a complying superannuation fund when requesting to rollover your existing super funds into your BT Super For Life account.
   
   
4. Add Personal Adviser (PDF 450 KB)*
   Use this form to add a new personal adviser to your BT Super for Life account.
   
   
5. Nomination of Beneficiaries (PDF 285 KB)
   Use this form to nominate the beneficiaries to receive your superannuation benefit in the event of your death.

Transactions

1. Personal Tax Deduction Notice* - Use this form to ad­vise whether a tax de­duc­tion will be claimed on per­sonal con­tri­bu­tions re­ceived in BT Su­per for Life on or af­ter 1 July 2007.
   
   
2. Contribution Splitting* - Use this form if you in­tend split­ting su­per­an­nu­a­tion con­tri­bu­tions with an el­i­gi­ble spouse. Please see at­tached form for de­f­i­n­i­tion of an el­i­gi­ble spouse.
   
   
3. Member Advice Fee* - Use this form if you wish to have a Member Advice fee deducted from your account.

Withdrawing funds

1. Withdrawal on Compassionate Grounds - use this form if you wish to withdraw money from your BT Super for Life account on compassionate grounds - you must apply through the Department of Human Services.
   
   
2. Financial Hardship claim - use this form if you are seeking an early release of your superannuation benefits on the grounds of severe financial hardship.
   
   
3. Withdrawal request - use this form to withdraw part or all of your investment in BT Super for Life.
   
   
4. Rollover to SMSF – use this form to request to transfer whole or partial balance of superannuation benefits to your self managed super fund (SMSF)
   
   
5. Customer Identification and Verification form – Individuals and Sole Traders – use this form if requested to provide customer identification under the Anti-Money Laundering (AML) and Counter Terrorism Financing (CTF) Act 2006.
   
   
6. Transfer to Kiwisaver - Use this form when you wish to close and transfer your entire superannuation benefit to your Kiwisaver Scheme.

Insurance

1. Customised Insurance – Application Form* – use this form for BT Su­per for Life Cus­tomised In­sur­ance for ap­pli­cants (for Death, To­tal and Per­ma­nent Dis­able­ment and Salary Con­tin­u­ance In­sur­ance cover).
   
   
2. Customised Insurance – Takeover Terms Application* -  this form can be used when a takeover of non BT Su­per for Life Cus­tomised cover to the BT Su­per for Life Cus­tomised cover is to oc­cur.
   
   
3. BT Super for Life – Insurance Opt-in Nomination - Use this form to opt in to receive Standard cover as soon as there is money in your account, and/or increase your Standard cover, subject to meeting the eligibility criteria without the need to complete any medical checks.
4. BT Alteration Request - You can use this form for the following changes:
   • change of Benefit Period (decrease) and/or Waiting Period (increase) for Salary Continuance Insurance only
     
   • increase or decrease of cover amount
     If you wish to increase cover, increase your Benefit Period or decrease your Waiting Period, you will need to complete this form along with the Personal Statement for underwriting assessment.

1. Rollover form* - Use this form to con­sol­i­date your cur­rent su­per funds into your BT su­per ac­count. Note you will need one form for each su­per fund you're rolling over from.
2. Super Choice form - Request your employer to make contributions to your BT Super account with a Super Choice form.
3. Letter of Compliance - Your previous fund may request that you provide BT's Letter of Compliance when rolling over. This letter states that your BT superannuation fund complies with applicable law.

Setting up

1. Nomination of Beneficiaries* - Use this form to nominate who you would like to receive your superannuation benefits in the event of your death. You can also make a nomination via our online portal
2. Change of Details Request - Use this form to change personal or account details such as your address. If you would like to advise of a change of name, gender or date of birth a printed and signed form needs to be provided.
3. Add Personal Adviser* - Use this form to add a new personal adviser to your BT Super account.

Transactions

1. Investment Choice form* - Use this form to switch funds from one investment option to another, or if you would like to choose the investment options where your future contributions and rollovers are to be invested. You can also make a change via our online portal.
2. Contribution Splitting* - Use this form if you intend splitting superannuation contributions with an eligible spouse. Please see attached form for definition of an eligible spouse.
3. Member Advice Fee* - Use this form if you wish to have a Member Advice fee deducted from your account.
   
4. Appoint an Authorised Representative* - Use this form to appoint someone to operate your account.

Insurance

Employee Insurance
Members who have joined BT Super through an Employer Plan

1. BT Super - Employee Insurance - Life Insurance Application* - Use this form to apply for insurance cover, or increase your current insurance arrangements under your superannuation plan.
2. Insurance Opt-up Application* - You can use this form if you are under age 70 with Essential Cover, or under age 35 with Tailored Death Cover, and you want to increase your insurance cover without providing medical evidence.

1. BT Super - Lifetime Insurance - Life Insurance Application* - Use this form to apply for insurance cover, or increase your current insurance arrangements under your superannuation plan.

1. Life Stage Insurance Application* - Use this form to apply for an increase in Death Only insurance cover on the occurrence of certain "Personal Events" or "Business Events".
2. Application to Reduce or Cancel Insurance Cover* - Use this form to reduce or opt-out of insurance cover under your superannuation plan.
3. Individual Insurance Consolidation* - BT offers consolidation of non-BT super and non-super insurance covers into the member fund.
4. PYS Inactivity Insurance Opt-in - Use this form to opt in to maintain insurance cover(s) if your account is, or becomes, inactive for a continuous period of 16 months.

Tax

1. Personal Tax Deduction Notice* - Use this form to advise whether a tax deduction will be claimed on personal contributions received in BT Super on or after 1 July 2007.

• Set up an Advice Fee Arrangement (PDF 135 KB)
  Use this form for if you have agreed with your client to set-up a new advice fee arrangement to have a one-off and/or ongoing advice fee deducted from their BT account. You can also use this form to amend or replace an existing advice fee arrangement on their BT account, if you have previously set-up an ongoing advice fee by completing an Advice Fee form or the Advice Fee section of their application form on or after 1 July 2013. This form must be completed and signed by you and your client. This form applies to clients in BT Classic Investment Funds, BT Investor Choice Funds, BT Investment Funds, BT Global Share Fund and BT Balanced Deeming Fund.
