BT Super for Life

Setting up

  1. Choice of Superannuation Fund Form (PDF 154 KB)

    Complete this form and hand it to your employer or payroll manager to ask them to pay your super into your BT Super for Life account.

  2. Rollover Form (PDF 302 KB)

    Complete this form if you wish to rollover your existing super funds into your BT Super for Life account. Simply complete the details of your existing super fund/s. You will need a new form for each fund you wish to rollover. After completing the form, attach a copy of certified ID. You can certify your ID at your local Westpac branch, Australia Post outlet or Police Station. Make sure you have a separate copy of certified ID for each form you complete. Then send the form/s and your certified ID to your existing super funds.

  3. BT Super for Life Compliance Letter (PDF 51 KB)

    Provide this letter as evidence of BT Super For Life being a complying superannuation fund when requesting to rollover your existing super funds into your BT Super For Life account.

BT Lifetime Super - Personal Super Plan

Setting up

    Your previous fund may request that you provide BT's Letter of Compliance when rolling over. This letter states that your BT superannuation fund complies with applicable law.

    Nominate BT Lifetime - Personal Super to receive your Superannuation Guarantee (SG) contributions, by giving this completed form to your employer. This form may be used instead of the 'Superannuation Standard Choice' form.

  5. Withdrawal Request

    This form is used to roll over your superannuation to another fund or, if you are eligible, make a withdrawal. If you require this withdrawal form, please call BT Customer Relations on 132 135.

  6. Advice Fee form (PDF 142 KB)

    Use this form if you have agreed with your financial adviser to set-up a new advice fee arrangement to have an ongoing advice fee deducted from your BT account. You can also use this form to amend or replace an existing advice fee arrangement on your BT account, if you have previously set-up an ongoing advice fee by completing an Advice Fee form or the Advice Fee section of your application form on or after 14 December 2011.

  7. Advice Fee Cancellation form (PDF 187 KB)

    Use this form to cancel a monthly ongoing advice fee arrangement set up on or after 14 December 2011 by using the Advice Fee form (or Advice Fee section of their application form). If you have not previously set up an ongoing advice fee for your BT account in this way, please contact your financial adviser to discuss your arrangement with them. This form can be completed by you, or your financial adviser associated with the monthly ongoing advice fee arrangement that is being cancelled (or a financial adviser authorised under the same AFSL).

    Use this form to appoint someone to operate your account.

Tax

    Use this form each time an employer contribution is made for a spouse or retained member account, and for all other types of contributions after the initial contribution.
  3. Direct Personal Member and spouse application form (PDF 179 KB)
    Use this application form if you are a spouse of an existing member and want to join the existing members employer sponsored plan within BT Lifetime Super – Employer Plan, or if you are a personal investor who wants establish an individual account with BT Lifetime Super – Employer Plan directly.
  4. Business Partner and Sole Trader application form(PDF 149 KB)
    Use this application form if you want to add a Business Partner and Sole Trader to an existing plan within the BT Lifetime Super – Employer Plan.
  5. Investment Choice/Switch Request (PDF 152 KB)
    Use this form to switch funds from one investment option to another or if you would like to specify the investment options where your future contributions and rollovers are to be invested. Unless you advise BT that you would like to specify your investment strategy, your investments will be placed in the default investment option selected by your employer.
