BT Investment Funds

Setting up

1. BTLink and BTOnline Registration (PDF 187 KB)
Use this form to register for BT Link (our automated telephone service) and BTOnline (our secure website).

2. Change of Details Request (PDF 98 KB)
Use this form to change personal or account details such as your address or your distribution options.
You must use this form for change of name, bank account details and new account signing authority.
Better yet, update your details online, log into BTOnline, or call 132 135.

3. Request Cheque Book Facility – s.21 form (PDF 86 KB)
Use this form to verify your identity so that your cheque book may be issued. Also include either your completed application form, or if you are an existing investor, a covering letter requesting a cheque book. Subsequent cheque books can be ordered by logging into BT Online or calling Customer Relations.

4. Additional Investment Request (PDF 130 KB)
Use this form to make additional investments. Alternatively, if you already have a direct debit arrangement in place, log in to BTOnline, use BTLink, call 132 135 or send funds via BPAY.

5. Switch Request (PDF 134 KB)
Use this form to switch funds from one investment option to another. Alternatively, if you already have a direct debit arrangement in place, log in to BTOnline, use BTLink, call 132 135.

6. Regular Investment Plan/Direct Debit Request (PDF 130 KB)
Use this form to set up or change a Regular Investment Plan that is funded from your bank account. The form can also be used to set up a direct debit arrangement. If you already have a direct debit arrangement in place, you can also set up or change a Regular Investment Plan if you log in to BTOnline or call 132 135. At least 7 working days notice is required to change a plan.

7. Payroll Deduction Authority for a Regular Investment Plan (PDF 85 KB)
Use this form to set up or change a Regular Investment Plan that is funded from payroll deductions (requires your employer to be able to pay your salary to multiple bank accounts).

8. Withdrawal Request
This form is used to roll over your superannuation to another fund or, if you are eligible, make a withdrawal. If you require this withdrawal form, please call BT Customer Relations on 132 135.

9. Flexi Payment Plan Request (PDF 129 KB)
Use this form to set up or change a Flexi Payment Plan. Alternatively, if you already have a Flexi Payment Plan in place, you can amend it by calling Customer Relations. At least 7 working days notice is required to change a plan.

10. Appoint an Authorised Representative (PDF 44 KB)
Use this form to appoint someone to operate your account.

11. Set up an Advice Fee Arrangement (PDF 135 KB)
Use this form if you have agreed with your financial adviser to set-up a new advice fee arrangement to have a one-off and/or ongoing advice fee deducted from your BT account. You can also use this form to amend or replace an existing advice fee arrangement on your BT account, if you have previously set-up an ongoing advice fee by completing an Advice Fee form or the Advice Fee section of your application form on or after 1 July 2013. This form must be completed and signed by you and your financial adviser.

12. Cancel an Advice Fee Arrangement (PDF 74 KB)
Use the form to cancel a monthly ongoing advice fee arrangement set up on or after 1 July 2013 by using the Advice Fee form (or Advice Fee section of your application form). If you have not previously set up an ongoing advice fee for your BT account in this way, please contact your financial adviser to discuss your arrangement with them. This form can be completed by you, or your financial adviser associated with the monthly ongoing advice fee arrangement that is being cancelled (or a financial adviser authorised under the same AFSL). 

BT Investor Choice Funds

Setting up
