Forms for advisers

  • Use this form to set up a New Adviser for BT Retail/Corporate Super or Margin Lending products or for changes to details of an existing Adviser.

  • Use this form to set up a New Dealer Group (Head Agent) for BT Retail/Corporate Super or Margin Lending products or to change details of an existing Dealer Group.

  • Use this form to transfer a BT Retail/Corporate Super or Margin Lending Advisers between Dealer Groups.

  • Use this form to transfer a single investor between BT Retail/Corporate Super or Margin Lending Advisers. This can occur between Dealer Groups or within a Dealer Group.

  • Use this form to transfer a multiple investor between BT Retail/Corporate Super or Margin Lending Advisers. This can occur between Dealer Groups or within a Dealer Group.

  • Use this form to request access to BT Adviser Exchange (BTAX) for BT Retail/Corporate Super or Margin Lending products.

  • Use this form for if you have agreed with your client to set-up a new advice fee arrangement to have a one-off and/or ongoing advice fee deducted from their BT account. You can also use this form to amend or replace an existing advice fee arrangement on their BT account, if you have previously set-up an ongoing advice fee by completing an Advice Fee form or the Advice Fee section of their application form on or after 1 July 2013. This form must be completed and signed by you and your client. This form applies to clients in BT Classic Investment Funds, BT Investor Choice Funds, BT Investment Funds, BT Global Share Fund and BT Balanced Deeming Fund.

  • Use this form to amend an existing Member Advice fee or to have a Member Advice fee deducted from your account.

  • Use this form to amend an existing Member Advice fee or to have a Member Advice fee deducted from your account.

  • Use this form to amend an existing Member Advice fee or to have a Member Advice fee deducted from your account.

  • Use the form to cancel a monthly ongoing advice fee arrangement set up on or after 1 July 2013 by using the Advice Fee form (or Advice Fee section of your client's application form). This form can be completed by you (or a financial adviser authorised under the same AFSL), or your client associated with the monthly ongoing advice fee arrangement that is being cancelled. This form applies to clients in BT Classic Investment Funds, BT Investor Choice Funds, BT Investment Funds, BT Global Share Fund and BT Balanced Deeming Fund.
