Forms for BT Lifetime Super – Personal Super

Use one of the forms below to arrange a transaction or to update the details of your investment.

Forms to arrange a transaction

  1. Nominate a Tax File Number (PDF 50KB)
    Use this form to nominate a tax file number and avoid penalties.

  2. Consolidate your super
    Consolidate your super funds quickly and easily using BT's Easy Rollover Tool. You simply provide your details once, and the Tool will automatically create all the forms you need to bring your super investments together.

    Alternatively you can use the Request to Transfer Form (PDF 165KB) to roll money invested in any previous superannuation funds into your BT superannuation account. Use a separate form for each previous fund.

  3. Super Choice Fund Form (PDF 120KB)
     To nominate BT Lifetime – Personal Super to receive your Superannuation Guarantee (SG) contributions, complete this form and return it to your employer. This form may be used instead of the 'Choice of Superannuation Fund – Standard Choice' form

  4. Letter of Compliance (PDF 30KB)
     If you are rolling money invested in any previous superannuation funds into your BT superannuation account, the previous fund may request that you provide BT’s Letter of Compliance. The letter states that your BT super fund complies with applicable law.

  5. Additional Contributions and Direct Debit Request Form (PDF 70KB)
     Use this form to make additional contributions. The form can also be used to set up a direct debit arrangement. Alternatively, if you already have a direct debit arrangement in place, log into BT Online, use BT Link or call customer relations to make an additional contribution. You may also arrange an additional contribution without a form by sending funds via BPAY.

  6. Switch Request (PDF 50KB)
     Use this form to switch funds from one investment option to another. Alternatively, log into BT Online, use BT Link or call customer relations to arrange a switch.

  7. Personal Tax Deduction Notice - for contributions on or after 1 July 2007 (PDF 167KB)
     Use this form to advise whether a tax deduction will be claimed on personal contributions received in BT Personal Super on or after 1 July 2007.

  8. Downward Variation of a Previous Tax Deduction Notice (PDF 163KB)
    Use this form if you have already provided us with a Personal Tax Deduction Notice for your personal contributions and you wish to reduce the amount stated in that notice.

  9. Contribution Splitting (PDF 130KB) Use this form if you intend splitting superannuation contributions with an eligible spouse. Please see attached form for definition of an eligible spouse.

  10. Eligibility to Contribute (PDF 70KB)
     Use this form to confirm that you are eligible to make superannuation contributions into your account.

  11. Life Insurance Application (PDF 137KB)
     Use this form to apply for insurance cover, or increase your current insurance arrangements under your superannuation plan.

  12. Personal Statement for Insurance Cover (PDF 90KB)
     Use this form if you answered ‘yes’ to any of the questions in the Health declaration section of the Life Insurance Application form (above).

  13. Questionnaires for Insurance Cover (PDF 64KB)
     Use this form if you have been directed to do so when completing the Personal Statement for Insurance Cover form (above).

  14. Application to Reduce or Opt-out of Insurance cover (PDF 73KB)
     Use this form to reduce or opt-out of insurance cover under your superannuation plan.

  15. Insurance Amendment (PDF 200KB)
     Use this form to amend your details for your Death only or Death and Total and Permanent Disablement (TPD) insurance cover, including your occupation and smoker status

  16. Withdrawal Request
    This form is used to roll over your superannuation to another fund or, if you are eligible, make a withdrawal. If you require this withdrawal form, please call BT Customer Relations on 132 135.

Forms to update your account details

  1. Change of Details Request (PDF 50KB)
     Use this form to change personal or account details such as your address. Alternatively, log into BT Online or call customer relations.

    If you would like to provide new bank account details or advise of a change of name, a printed and signed form needs to be provided.

  2. BT Link and BT Online Registration (PDF 80KB)
     Use this form to register for both BT Link (our automated telephone service) and BT Online (the secure section of this website). Alternatively, call customer relations.

  3. Appoint an Authorised Representative (PDF 40KB)
     Use this form to appoint someone to operate your account.

  4. Nomination of Beneficiaries (PDF 70KB)
     Use this form to nominate who you would like to receive your superannuation benefits in the event of your death.
