Forms for BT Investment Funds

Use one of the forms below to arrange a transaction or to update the details of your investment.

Forms to arrange a transaction

  1. Additional Investment Request (PDF 60KB)
    Use this form to make additional investments. Alternatively, if you already have a direct debit arrangement in place, log into BT Online, use BT Link or call Customer Relations to make an additional investment. You may also arrange an additional investment without a form by sending funds via BPAY

  2. Switch Request (PDF 50KB)
    Use this form to switch funds from one investment option to another. Alternatively, log into BT Online, use BT Link or call Customer Relations to arrange a switch.

  3. Withdrawal Request (PDF 60KB)
    Use this form to make a withdrawal. Alternatively, log into BT Online, use BT Link or call Customer Relations to arrange a withdrawal.

  4. Regular Investment Plan/Direct Debit Request (PDF 60KB)
    Use this form to set up or change a Regular Investment Plan that is funded from your bank account. The form can also be used to set up a direct debit arrangement.

    If you already have a direct debit arrangement in place, you can also set up or change a Regular Investment Plan if you log into BT Online or call Customer Relations. At least 7 working days notice is required to change a plan.

  5. Payroll Deduction Authority for a Regular Investment Plan (PDF 50KB)
    Use this form to set up or change a Regular Investment Plan that is funded from payroll deductions (requires your employer to be able to pay your salary to multiple bank accounts).

  6. Flexi Payment Plan Request (PDF 50KB)
    Use this form to set up or change a Flexi Payment Plan. Alternatively, if you already have a Flexi Payment Plan in place, you can amend it by calling Customer Relations. At least 7 working days notice is required to change a plan.

Forms to update your account details

  1. Change of Details Request (PDF 60KB)
    Use this form to change personal or account details such as your address or your distribution options. Alternatively, log into BT Online or call Customer Relations.

    The following changes require a printed and signed form to be provided;
    • change of name,
    • new account signing authority, and
    • new bank account details (for the crediting of withdrawals and distributions or to establish a direct debit arrangement).

  2. BT Link and BT Online Registration (PDF 80KB)
    Use this form to register for both BT Link (our automated telephone service) and BT Online (the secure section of this website). Alternatively, call Customer Relations.

  3. Request Cheque Book Facility – s.21 form (PDF 80KB)
    Use this form to verify your identity so that your cheque book may be issued. Also include either your completed application form, or if you are an existing investor, a covering letter requesting a cheque book. Subsequent cheque books can be ordered by logging into BT Online or calling Customer Relations.

  4. Appoint an Authorised Representative (PDF 40KB)
    Use this form to appoint someone to operate your account.
