Step 1 – Mandatory documents you should complete and return to us

The checklist on page 1 of this form will help you complete all necessary sections and identify any other forms you may need to complete. - BT

Step 2 – Optional forms
Refer to the checklist in the application form to determine which forms you may need to complete.
- Direct Debit Request (DDR) Form (PDF 75KB) - to enable BT Margin Lending to debit and/or credit your account.
- Loan Refinancing Form (PDF 75KB) – to transfer your existing margin loan to BT Margin Lending for full or partial refinancing.
- Managed Fund Application and Regular Gearing Form (PDF 75KB) - to make initial managed fund applications and/or to commence regular gearing. If you are commencing a regular gearing plan and require equity contribution, please ensure you complete the DDR form as well.
- Australian Standard Transfer Form (ASTF) (PDF 25KB) - to transfer existing managed fund investments to your BT Margin Loan. You need to complete a separate ASTF for each individual fund, even if they are with the same fund manager.
- Lodging Shares Form (PDF 65KB) - to transfer existing shares to your BT Margin Loan.
- Name Correction and Indemnity Request Form (PDF 65KB) - to correct discrepancies in the registration details of existing issuer-sponsored share holdings to be applied to your BT Margin Loan.
Step 3 – Ensure you provide the necessary collateral for the margin loan
- a cheque to fund part of your investments
- details of existing investments (refer to optional forms above)
Step 4 – Mail your completed original Loan Application Form and any Optional Forms to:
