• BT Super for Life
  To take out or increase your insurance cover through BT Super for Life you need to log in to Westpac Online (opens in a new window). Go to the Superannuation section and follow the prompts to apply for or increase your insurance cover.
• BT Lifetime Super - Personal Super (PDF 137KB)
  Use this form to apply for insurance cover, or increase your current insurance arrangements under your superannuation plan.
• BT Lifetime - Personal Super Life Stage Insurance (PDF 270KB)
  Use this form to increase your level of Death only or Death and TPD cover when certain ‘personal events’ or ‘business events’ occur.

  Use this form to apply for insurance cover, or increase your current insurance arrangements under your superannuation plan.
