Forms to arrange an investment transaction

1. Cash Advance Request (PDF 169KB)
   Use this form to advance funds out of your BT Margin Loan. Alternatively, log into BT Online (opens in a new window) or call Customer Relations on 1800 816 222.
   
   
2. Direct Debit Request Form (PDF 306KB)
   Use this form to set up or change a direct debit arrangement so that you can credit or debit your bank account from your margin loan. You can use this form to request either a monthly interest deduction, a one-off deduction or regular deductions. Once your direct debit request has been received you can call Customer Relations on 1800 816 222 to request a transaction.
   
   
3. Managed Funds Transaction Form (PDF 346KB)
   Use this form to complete a managed fund transaction. Transactions include initial and additional applications, redemptions, distribution amendments and establishing or amending regular gearing.

Forms to vary your security or loan

4. Lodging Shares Form (PDF 235KB)
   Use this form to lodge shares and other listed securities as security on to your margin loan. This includes lodging third party security.
   
   
5. Lodge Managed Funds (PDF 288KB)
   Use the Australian Standard Transfer Form (ASTF) to lodge managed funds as security on your margin loan. This includes lodging third party security. Completed forms must be returned by post and not faxed.
   
   
6. Switch Form A: From Standard to Margin Lending_Online (PDF 122KB)
   Use this form to switch your existing BT Margin Lending Standard Margin Loan to the BT Margin Lending_Online loan facility.
   
   
7. Switch Form B: From Margin Lending_Online to Standard (PDF 87KB)
   Use this form to switch your existing BT Margin Lending_Online loan facility to a standard BT Margin Lending Standard Margin Loan.

   Use this form to transfer securities from your margin loan or to change the beneficial ownership of your securities.

   Change the beneficial or legal ownership of a managed fund or listed security.
   
   
10. Loan Refinance Form (PDF 312KB)
    Use this form to transfer your existing margin loan with another provider to BT Margin Lending, for either a full or partial refinance.
    
    
11. Third Party Security Provider Application Form (PDF 352KB)
    Use this form when security on a BT Margin Loan Facility is being provided (owned) by someone other than the borrower.

    Use this form when you would like to apply for a credit limit increase or where BT Margin Lending has requested an updated financial assessment from you.
    
    
13. Credit Limit Decrease Form (PDF 167KB)
    Use this form where you would like to decrease the limit on your BT Margin Loan Facility.
    
    
14. Loan Repayment Form
    This form is used to payout and/or terminate your BT Margin Loan. If you require this form, please contact our BT Customer Relations on 1800 816 222 between 8.00am and 6.30pm Sydney time.

    Use this form to add or amend the contact details for a borrower, third party security provider or guarantor. Alternatively, log into BT Online (opens in a new window).
    
    
16. Name Correction and Indemnity Request Form (PDF 195KB)
    Amend any discrepancies in the registration details of an Issuer sponsored share holding, so that the shares can be lodged as security to your margin loan. Registration details at the share registry must exactly match those on your margin loan.

19. Proof of Identification Guide (PDF 350KB)
    Use this guide when a borrower, guarantor or authorised representative on a BT Margin Loan Facility needs to be identified as part of the Anti-Money Laundering and Counter-Terrorism Financing (AML/CTF) Act 2006. This is usually required where any of these parties are not existing Westpac customers.
