Use one of the forms below to update the details of your investment. To either arrange a withdrawal or to switch from one investment option to another, please provide instructions in a letter (no form is required).

Tax file number declaration (PDF 1MB)
Use this form to lodge your tax file number declaration.

Change of Details Request (PDF 50KB)
Use this form to change personal or account details such as your address. Alternatively, log into BT Online (opens in a new window) or call customer relations.
If you would like to provide new bank account details or advise of a change of name, a printed and signed form needs to be provided.

Nomination of Beneficiaries (PDF 127KB)
Use this form to nominate who you would like to receive your superannuation benefits in the event of your death.

Use this form to register for both BT Link (our automated telephone service) and BT Online (opens in a new window) (the secure section of this website). Alternatively, call customer relations.

Appoint an Authorised Representative (PDF 40KB)
Use this form to appoint someone to operate your account.

Letter of Compliance (PDF 30KB)
If you are rolling money invested in any previous superannuation funds into your BT superannuation account, the previous fund may request that you provide BT's Letter of Compliance. The letter statess that your BT super fund complies with applicable law.

Switch and Change of Income Payment Details Form (PDF 160KB)
Use this form to switch between Investment Options within the Plan and change income payment details. Alternatively, call customer relations.

Withdrawal Request
This form is used to roll over your superannuation to another fund or, if you are eligible, make a withdrawal. If you require this withdrawal form, please call BT Customer Relations on 132 135. -
Advice Fee form (PDF 206KB)
Use this form if you have agreed with your financial adviser to set-up a new advice fee arrangement to have an ongoing advice fee deducted from your BT account. You can also use this form to amend or replace an existing advice fee arrangement on your BT account, if you have previously set-up an ongoing advice fee by completing an Advice Fee form or the Advice Fee section of your application form on or after 14 December 2011. -
Advice Fee Cancellation form (PDF 148KB)
This form can only be completed for members who have already set-up a monthly ongoing advice fee arrangement with their financial adviser on or after 14 December 2011 by completing an Advice Fee form (or Advice Fee section of their application form). If you have not previously set-up an ongoing advice fee for your BT account in this way, please contact your financial adviser to discuss your arrangement with them. This form can be completed by you, or your financial adviser associated with the monthly ongoing advice fee arrangement that is being cancelled (or a financial adviser authorised under the same AFSL).
